No matter where you are in your caregiving journey — starting to plan; taking care of a family member in your home, in a facility, or from a distance; or managing end-of-life caregiving responsibilities — having resources at your fingertips will make the process easier.
AARP developed this family caregiver guide with you, the caregiver, in mind and as a starting point to help you find the services and support you might need throughout your journey.

Caregiver Support
New York State Caregiving & Respite Coalition (585-287-6393)
Offers training and education to increase caregivers’ access to respite resources, including respite care vouchers.
Catholic Charities Senior & Caregiver Support Services (518-372-5667)
Offers consultations and support groups for caregivers of adults age 60-plus residing in upstate New York.
Finances

Offers training and workshops to educate older adults about their federal and state program eligibility. |
Office of Temporary and Disability Assistance (518-473-1090)
Helps residents check eligibility and apply for tax credits, health insurance and aid programs such as Supplemental Nutrition Assistance and Home Energy Assistance.
State Paid Family Leave (844-337-6303)
Paid family leave resources available to eligible employees who may seek time off work to care for a sick or older family member, newborn or adopted child.
Legal
Center for Elder Law and Justice (716-853-3087)
Offers free civil legal services to older people, individuals living with disabilities and some low-income residents within Western New York.
Legal Aid Society (212-577-3300)
Provides support and advocacy to workers, individuals living with disabilities and those seeking legal advice for issues related to gender, race or sexuality, among other services. An elder law practice helps New Yorkers dealing with age-related legal problems.
Health
Health Insurance Information Counseling and Assistance Program (800-701-0501)
Counselors answer residents’ questions about Medicare, Medicare Advantage programs and prescription drug coverage, Medigap and other health and long-term care insurance.
State Department of Health Medicare Savings Program (800-541-2831)
Helps low-income residents cover their Medicare premiums
and may offer additional financial support.
State Official Health Plan Marketplace (855-355-5777)
Marketplace for low-cost health plans offers eligible residents financial assistance based on income.
Housing and Care Facilities
JASA (212-273-5272)
Operates several affordable housing properties and maintains a licensed home care arm. Offers integrative services including free legal services, health and mental health services, home-delivered meals, social programming at senior centers and community trainings on elder abuse, peer health support, caregiver assistance and more.
Long-Term Care Ombudsman (855-582-6769)
Provides consumer protection advocacy services to residents of long-term care facilities such as nursing homes, assisted living facilities, specialty care facilities and boarding homes. Anyone can use an ombudsman to report a concern and get help resolving problems, including residents, residents’ family members and friends, and long-term care facility employees.

Provides programs and services to older adults, focused on caregiving, health, housing, employment and elder abuse, among other aging-related topics.

Helps older adults, individuals living with disabilities, caregivers and loved ones understand and access available resources and long-term support services such as home care, as well as basic necessities such as transportation and meal services.
SAGE NYC (212-741-2247)
Offers innovative services and programs, from arts and culture to health and wellness, and much more to LGBT older people throughout New York.
Selfhelp Community Services (212-971-7600)
Provides home care and community- based services for older adults, frail and vulnerable New Yorkers, including housing, home care, health care, social services and community- based programs. Offers training programs for health care workers and operates North America’s oldest and largest program providing comprehensive services to Holocaust survivors.
Western New York Integrated Care Collaborative (716-431-5100)
Network of government agencies and community organizations offers training services and cost-effective education and community programs/ services for older adults, people with disabilities and their caregivers.
End of Life
Hospice & Palliative Care Association of New York State (518-446-1483)
Provides patients and their families resources to help make better choices regarding serious illness and end-of-life care.
